Cancellation Policy
We understand that there are times when you must miss an appointment due to emergencies or personal obligations. However, this prevents our office from providing care to another patient needing treatment. This is because we do not double book patient appointments.
Office Visit (New or Followup)
Cancellation/rescheduling requires a 24-hour notice. If cancelling/rescheduling after a 24-hour notice or on the day of appointment, Bellevue Headache Clinic reserves the right to charge your credit card $100 for NEW patients. Established patients will be billed $50 for a late cancellation.
No Shows
50% of the appointment fee ($200 for a 60 min appointment/$250 for a 90 min appointment) will be charged.
